Online Application

  • Applying online is easy! Fill out the online form, then download/distribute the Pastoral and Personnel recommendation forms.
  • Your credit card will be charged the $25 application fee.
  • We accept Visa, MasterCard, Discover, American Express, and PayPal. All credit card information is encrypted and secure for your safety.

Printed Application

  • Download the Application Packet and other forms to your computer for printing.
  • Mail the materials to Providence along with a check for your $25 application fee.
